Can anyone point us to a website or textbook that outlines proper
positioning for full length spine films for scoliosis surgery
planning. 14x36 image is used, but taking into consideration proper
shielding, how much of the pelvis is needed? Some say show only the
tops of the crests, others show as far as the hip joint. We could also
use detailed information on the AP (R/L) bend films also for surgery
planning. Centering, rationale, where to flex, the direction to flex
etc. Seems none of the sites so far go into this much detail.

I recommend a textbook: "Campbell's Operative Orthopaedics".
As for the pelvis, top of the crests and hip joint facilitate an
estimation of skeletal maturity, which is of prime importance when
considering mode of treatment. Look for "Risser's sign" and "hip epiphisis".
Bending films are performed to see whether deformity is correctable or
not. It is not important at which level patient bends the trunk, but the
extent of bending (the closest to fall). Anyway, they are not used by
all spine surgeons.
